When Symptoms Don’t Fade: Persistent Cognitive and Learning Problems After TBI

Most children who sustain a concussion recover within days to a few weeks. But a significant minority do not, and for these children the lingering effects show up not as headaches or dizziness but as difficulty thinking, remembering, and learning. Understanding how often this happens—and what it looks like—can help parents recognize when a child needs more than rest.

How often symptoms persist

Estimates vary by how researchers measure it, but a substantial share of children develop persistent post-concussion symptoms after a mild TBI. Reported rates range widely, from a few percent to as high as a third of cases, with many studies converging on roughly 10 to 30 percent. In one comparison, nearly 25 percent of children with mild TBIs fell into high-symptom groups—either persistent or slow-resolving—compared with only about 5 percent of an injury-control group. Researchers increasingly describe persistent post-concussion syndrome in children as both prevalent and vastly underdiagnosed.

Cognitive symptoms outlast the physical ones

One of the most important findings for parents is that the visible, physical symptoms of a concussion often fade before the cognitive ones do. Studies tracking children over time have found that while somatic complaints such as headache and nausea tend to resolve, cognitive symptoms can persist for as long as 12 months after injury. In other words, a child may look fully recovered while still struggling to keep up in the classroom.

What the cognitive effects look like

After a mild TBI, the cognitive skills most commonly affected include attention and concentration, short-term and working memory, executive functions such as planning and organization, and processing speed. A child may become easily distracted, lose track of multi-step directions, take longer to complete work, or have trouble keeping pace in conversation. Because these difficulties are subtle and invisible, they are frequently mistaken for laziness, defiance, or a new learning disability rather than being connected back to the injury.

Who is at higher risk

Not every child faces the same risk of lingering problems. A prior history of concussion, a pre-existing psychiatric condition, and limited support at home are all associated with slower recovery. A “complicated” mild TBI—one with abnormalities visible on imaging—also raises the odds of persistent symptoms.
