Most adults who sustain a concussion recover within days to a few weeks. But a significant minority do not, and for them the lingering effects often show up not as headaches or dizziness but as difficulty thinking, concentrating, and remembering. Understanding how often this happens—and what it looks like—can help you recognize when your symptoms deserve more than rest and reassurance.
How often symptoms persist
Persistent problems after mild TBI are more common than the word “mild” implies. In one large population study, nearly half of adults—about 48 percent—still reported four or more post-concussion symptoms a full year after injury, and roughly 10 to 11 percent showed measurable impairment in cognitive and overall functioning at that one-year mark. Other reviews estimate that up to 30 percent of adults experience prolonged symptoms, and about 15 percent develop post-concussion syndrome.
Cognitive symptoms outlast the physical ones
One of the most important findings for patients is that the physical symptoms of a concussion often fade before the cognitive ones do. Studies following adults for as long as four years after a mild TBI have found that while somatic and emotional complaints tend to resolve over time, self-reported cognitive symptoms can persist and even become the dominant long-term problem. In other words, you may look and feel physically recovered while still struggling to think as clearly as you once did.
What the cognitive effects look like
After a TBI, the cognitive skills most commonly affected include attention and concentration, short-term and working memory, executive functions such as planning and organization, and processing speed. You might find yourself losing track of conversations, forgetting why you walked into a room, needing far longer to finish tasks that used to be automatic, or feeling mentally exhausted by the end of the day. Because these difficulties are invisible, they are easily dismissed—by others and sometimes by yourself—as stress, aging, or simply not trying hard enough.
Who is at higher risk
Not everyone faces the same risk of lingering problems. A prior history of concussion, pre-existing anxiety or depression, older age, and limited support are all associated with slower recovery. A “complicated” mild TBI—one with abnormalities visible on imaging—also raises the odds. Knowing these risk factors helps you and your clinicians decide when closer follow-up is warranted rather than waiting and hoping symptoms will simply pass.
